Based on what we have covered in this course to this date, and a review of the literature, select one of the twenty-seven social determinants of health listed in the Kaiser Foundation chart presented above, which you feel impacts your community the most in terms of health disparities, on which you think a local nonprofit organization could work to improve. Provide an overview of a program you would recommend this nonprofit organization sponsor to reduce this problem in your community.

Instructions and Rubric:1. This is an individual assignment consisting of a 5-page maximum paper, including the title page, text, and references page.2. Identify the Health Disparity of Interest in your Community, and the name of a nonprofit organization from who you might seek assistance in this matter. This can be a national, state, or local nonprofit organization.3. Provide an overview of a program you would recommend the nonprofit organization undertake to work to reduce this disparity. Include an explanation of the problem, an overview of the program, and how this program could reduce the problem by noting the expected outcome of the program.4. Provide at least ten references.5. Use proper grammar/spelling/and APA format. Include an introduction and conclusions section.
